MRET and Re-admissions investment plans for 2017/18

The table below shows the CCG’s planned investment of £7,089,000.  The Commissioners have directed this funding to where it will have most impact in the local healthcare system.  These plans are developed to reduce emergency admissions, with the aim being to improve the patient’s recovery through earlier discharge, to support our patients remaining in their normal place of residence via enhanced community rehabilitation and reablement and avoiding re-admission to hospital.
